Subscriber LTV with one-time spillover

Full hybrid unit-economic LTV. Practice computing per-cohort total revenue (subscription charges + one-time orders) for subscriber customers, dividing by the cohort's subscriber count, and producing the per-subscriber blended LTV by signup-month cohort. The metric every subscription operator reports to the board as 'subscriber LTV including one-time spillover'.
