Days-in-AR by payer — open claims aging

First multi-CTE pipeline in this path: an open-AR CTE (filter + date diff) feeds an aggregation per payer. Days-in-AR is the canonical CFO-level metric for AR health; combined with open dollar amount per payer, it tells the story of which payers are slow-paying vs which have small AR balances.
